Many owner-operators tie it to a published index (like the DOE weekly average) or negotiate a flat percentage with a broker or shipper. If you're not sure, 0% is a safe default — you can always add it back in once you know the convention for that lane or customer.
Detention pay, layover fees, extra stops, tarping, or any other agreed add-on beyond the base linehaul rate. These should be itemized separately on the actual invoice too, not folded into the per-mile rate.
